First Steps Preschool - Enrollment

Please download, fill out, and print the following forms for enrollment.

Identification & Emergency Information (Lic 700)
Pre-Admission Health Evaluation (Lic 702)
Physician's Report (Lic 701)
Parents’ Rights (Lic 995)
Personal Rights (Lic 613A)
